SHFM Patient Centered Medical Home

 

NCQA's Patient Centered Medical Home is a government manifesto as a result of Health Care Reform. The goals:

1. Improve quality of care by obtaining maximized results through evidence based medicine

2. Reduce unneccesary/redundant costs spent on fragmented healthcare each year

Resulting in better access to health care, increased satisfaction with care, and improved overall health.

 

As part of a Patient Centered Medical Home, you will be:

• Asked to pick/assigned a Primary Care Physician
• Asked to review your medications at every visit
• Screened for important health issues that may not yet be diagnosed (including mental health)
• Asked to assist in updating your medical record to include recent test results and information from specialist and other providers
• Informed of coordinated care across all providers for health screenings and acute care
• Given a personal care plan and health goals
• Cared for using Evidence Based Guidlines (nationally recognized standards of medicine)
• Provided with 24 hour access to our physicians
• Provided with same day appointments when you are sick
• Notified of abnormal test results that your Physician has ordered

At South Hills Family Medicine, we feel there is an aspect of patient responsibility to maximize your satisfaction with our practice and your health care.  We ask that you:

• Make your doctor aware of your entire medical history
  
• Keep all appointments as scheduled.  If you are unable to keep an appointment please call 24 hours in advance
• Tell your doctor of all medications, vitamins, and supplements you take
  
• Take all medication as prescribed or call your doctor if you plan to stop or change the way you are taking your medicine
• Consult my doctor before making appointments with a specialist or visiting an Urgent care or Emergency room (unless it is a true emergency)
• Contact my doctor within 48 hours of emergency room visits to schedule neccessary follow up appointments
