If you are a new patient, this will be provided to you at your first visit.

For our existing patients, you will be given a letter containing this code or you can call the office.  Please feel free to use the suggestion box on our homepage to request help with the patient portal.

To begin:

You will be prompted to fill out some basic personal information, as well as create a login name and password.  Please enter your name and date of birth exactly as it appears in our records system.  You will be able to make changes to your personal information once your portal account has been activated.  Please be as thorough as possible when filling out your personal information and medical history.  This will help us provide you the best medical care possible.
